Navigating Medical Bias in Maternity Care: How a Doula Supports Your Voice

Medical bias in maternity care is a documented reality: CDC data show about 1 in 5 women report mistreatment during maternity care, with higher rates among Black women. A doula cannot change hospital policies or replace your care team, but a doula can help you prepare questions, understand your options, and feel supported as you communicate with providers. Black Pearl doulas are trained in advocacy, cultural competency, and trauma-informed support.

What does the data say about mistreatment in maternity care?

In a 2023 CDC survey, about 1 in 5 women (20%) reported mistreatment while receiving maternity care, including violations of physical privacy, being shouted at or scolded, receiving no response to requests for help, and being threatened with withholding treatment. Mistreatment was reported by about 30% of Black women and 29% of Hispanic women, and nearly half of all women (45%) held back from asking questions or sharing concerns.

These numbers describe a systemic problem, not a personal failing. Knowing the data can help you plan how you want to show up for appointments and birth.

What is respectful maternity care?

Respectful maternity care means care that honors your dignity, privacy, and consent, where you are informed, heard, and involved in decisions. It includes explaining options, asking before procedures, and responding when you ask for help. Respectful care is the standard every family deserves, and a doula can help you recognize and ask for it.

How can a doula help you prepare for appointments?

Preparation turns anxiety into action. Your doula can help you write down your questions, practice how you want to raise concerns, and decide what you want to ask first. Learn how to prepare questions for your prenatal appointments and how to write a birth plan for step-by-step guidance.

How does a doula support you during labor?

During labor, your doula stays with you continuously, offering comfort measures and emotional support, and helping you process information from your care team so you can make informed decisions. A doula does not make medical decisions for you and does not speak over your care team; the doula's role is to support your voice, not replace it.

What if you feel unheard during an appointment or birth?

If something feels off, you have options. You can ask for clarification, request more time, ask who else can help, or ask for a second opinion. Bringing a support person, writing down your concerns, and asking for a patient-advocacy or nursing supervisor when needed are all reasonable steps. Your doula can help you organize what you want to say and stay calm while you say it.

What are the limits of doula advocacy?

Doulas are not licensed healthcare providers. They do not diagnose, treat, prescribe, or give medical advice, and they cannot guarantee any birth outcome. What a doula can do is help you prepare questions, support informed communication, help you understand your options, and encourage you to discuss medical decisions with your physician, midwife, or other licensed provider.

Can a doula speak for me?

A doula's job is to help you speak for yourself. With your permission, a doula can help communicate your questions or preferences to your care team, but the goal is always to make sure your voice is heard, not to speak over you.
